Vasectomy

Patients can self refer themselves for an NHS funded Vasectomy by contacting one of the following 2 GP practices in Coventry for advice & support:

Hearing Test for Patients aged over 55

All patients 55 years and over can now self-refer for a hearing test. They can call IDC Audiology directly on 01530 239195 and the office will go through the referral form and arrange any appointments.
Patients under 55 who require a hearing test must still be referred directly to Audiology at Warwick Hospital by a GP (or their preferred/local NHS hospital dept).

We support patients who are experiencing non-medical challenges, which are in turn having an effect on their overall health and well-being. We help you explore what matters to you and link you in with services that may be better suited to your needs.

We cannot solve your problems for you, but together we can help you take a step in the right direction. We are able to support you over a series of 6 sessions.

Financial issues
Accessing support as an unpaid carer
Dementia support
Bereavement
Linking you in with groups and courses to support your mental well-being
Issues with housing
Finding a purpose and joining local groups

To self refer to the Social Prescribing service, please email the SP team on cwicb.kw.socialprescribing@nhs.net – please note, there maybe a short waiting list for self referrals to the SP service.
